There's a very useful account of the why's and how's of code review in

	_Handbook of Walkthroughs, Inspections, and Technical Reviews_
	Daniel P. Freedman and Gerald M. Weinberg
	Little, Brown (ISBN 0-316-292826)

Everything you want to know about various alternative approaches, and then 
some.  As far as I know, the third edition (1982) is the most recent.
