We've used P2C (PL/M to C Translator) by Frank Korzeniewski and been pleased.
It is a commercial package, the advertized price is $5000 for a site license,
other arrangements may be available.  Several other packages were evaluated,
this was the best.

The only thing it doesn't handle at the moment (V1.4) is conditional
compilation.  I don't have the address or phone number in front of me; I know
he's indicated a uunet address in some of his documentation.  Hopefully
someone in netland can provide that information?
