I haven't tried this specific application, but I'd look at the "target" function in HyperCard which can return various properties of objects, such as the name of the button which was just clicked. You could get the name or number of the button which received a mouse-down and update a corresponding count in a container; that is, have a container which holds a summary of the responses for all the questions - each line in the container could be for a different question (treat it like an array).
