If you get Computing Systems, check out the article starting on p. 103 by Brian Kernighan and Christopher Van Wyk titled _Page_Makeup_by_Postprocessing_Text_Formatter_Output_. At a glance, it looks like it addresses many of troff's formatting problems (widows, orphans, and page justification) and it requires one hack to troff. If there is interest, I can summarize the article when finished (please give me a week-or-so, I have a couple of other things to do).
