I've had a couple of people look at my code (it's a straight INT 33h mouse interface) and no one has been able to spot a problem. From the symptoms, it appears to be very closely related to the mouse driver version and the speed of the computer it's running on. Anyway, a patched version that works around this problem has just been posted to comp.binaries.ibm.pc, called SOLITILE 1.3 and it should fix your problem. Sorry for the inconvenience.
