Do this: 1) Find a disk that you know is bad. 2) Attempt to format it in the Finder (I was using 6.0.3 on an SEx). 3) When the 'Initialization failed.' message appears, type anything (don't hit return). If you don't have a bad disk, you can initialize a good disk, and eject it with a paper clip before it you confirm the process. (I don't recommend doing this, just throwing out possibilities).
