The "original" source for this info, from an icon-editor-writer's point of view, is what comes with Apple's own Icon Editor from APDA (which I will not evaluate here :-). A better reference for the format of the stuff inside icon files is Apple II File Type Note $CA: Finder Icons File (July 1989).
