These can be obtained from SunRiver.  Anyone in need is welcome to email

Another alternative appears to be the UnTerminal.  Address avail as above.
This product allows you to multiplex (perhaps not the correct term) up to
2 VGA colour or 4 mono monitors/keyboards per host card.  We're hoping to 
do an eval on them soon.
