It was something in the order of 8 pages - but unfortunately, my hardcopy was accidentially trashed. If someone could pull up their lucky copy and post it, that would not only be great for me - but all. Just think, if you make a short call each time on a different company, you could find the cheapest one with- in a few... oh, never mind :).
