Your request was for documents which have had their structure
changed.  After a recent edit session of the ISO/IEC JTC1/SC21
the following documents were produced:

N-3874  ISO/IEC 9595 Common Management Information Service Definition
N-3876  ISO/IEC 9595/DAD1 
N-3877  ISO/IEC 9595/DAD2
N-3875  ISO/IEC 9596 Common Management Information Protocol Specification
N-3878  ISO/IEC 9596/DAD1

There is probably a newer revision of the Structure of Management Information
document, but I won't be able to contact the chap who has that until Friday.
When I get it I will post the reference here.

In any case contact Omnicom in Vienna, VA for copies of the documents.
