No. The majority of the errors come from people not following the
instructions.

Consider the simple instruction:

	Fill in questions 1, 2, 3 and leave 4, 5, 6 alone and
	return the entire form to me.

about 15% of the people "change the answers to 4,5 and 6 to
something wrong.

I dont know how much more simple you can make it than "leave #4 alone"
