When I need a modem cable, I use an old trick -- I buy an Apple IIe -> printer
(ImageWriter II) cable. Yup, a printer cable. You connect it "backwards", with 
the 8-pin connector going to the Macintosh, and the 25-pin connector going to
the modem.
 
Or, take a look at the Apple technical notes if you need pinouts. The 8-pin
connectors are hard to get, though.
