You didn't say which exact disk model and controller you had.  This info
is for the 9720-1230 (the 1.2GB Sabre) with the XY 753 or the 7053 (aka
SMD-4) controller:

back of drive
1,2,8,9 on [left] rest off [right]

top of drive
starting from front to back

sw1
1,2,3,4 unit id #, not necessary if front panel in place
5 off [b/8 to 8]
6 on [no write protect]
7,8,9,10 off

sw2
1,2,3 on
4,5 off
6,7,8,9,10 on

jumpers installed for swp1 and swpd [sweep disable]
