After some scrounging, I've gotten the ordering information
for the AT&T UNIX System V Interface Definition (SVID),
Issue 3.  Issue 3 == UNIX V.4 despite the confusing naming.

Ordering information can be obtained from AT&T at +1 (800) 432-6600.
There are 4 volumes to the set.

Here is the "select code" information:

Disclaimer:  I don't work for AT&T and am posting this only to help others
   who are also looking to get copies of the SVID.  Getting product ordering
   information from AT&T can be difficult due to their size...
