I need to write some worksheets for WordPerfect 5.0 (and since I don't
want to reinvent the wheel again), I was wondering whether anybody has
any forms already available which I could easily (with some minor and
even 1/2 major changes) transform into the format I want).

(PS:	If it isn't too clear, what I'm asking for are prearranged forms
	in which I could enter the data at specific fields on the screen
	and the data would be saved using the above format and can be
	thus retrieved (hopefully) next time for modification).
