As you know, some of us have been investigating the possibility of
getting some kind of group insurance plan for those of us who are
independant consultants and have no company plan.  A broker has asked
me to circulate the folowing questionaire to determine interest in
various sorts of plans and to get a rough idea of the probable
participation in each.  This will allow them to find the best plan for
our needs.  Please fill in the bottom portion of the message and return
it to darcy@druid.  I will tally the results and submit them to the broker.

NOTE: I won't be submitting actual names to the broker so you don't
have to worry about getting calls from insurance salesment into the
21st century.

To the Associates :

We are currently looking into the availability of an
association group insurance package. In order to assess the
level of interest in such a plan please take a moment to
respond to this notice.

The plan will offer a choice of benefits including Life
Insurance, Extended Health Care (prescriptions and major
expenses), Dental Insurance and Disability Insurance.

The actual levels of coverage to be offered and the costs
have not yet been determined. These will depend upon the
amount of interest expressed.

Please complete the information section and return it
as soon as possible.
