Cleanroom software development is also discussed briefly in 
  [5] Tom DeMarco, _Controling Software Projects_. Yourdon Press, NY, 
          1982. Chapter 22, "Zero Defect Development" pp 226-232.

Unfortunately, DeMarco slipped and did not cite _his_ sources for 
the "Zero Defect" approach, which looks to me to be identical or similar
to the "cleanroom" approach.
