Intel documents their object format in "8086 Relocatable Object Module
Formats," document 121748, available for $15 plus shipping from Intel.
In the US and Canada call 800-548-4725, other countries call your local
Intel office.

Having said that, be aware that Microsoft has added all sorts of incompatible
extensions to the OMF, so if you want to create or decode Microsoft object
files, you'll have to get documentation from Microsoft.
