TempOptilnnnn files are created by SIM TuneUp. It's not a virus, it's not a bug (maybe a misfeature), it's not a problem. If you have a locked file that is fragmented, when TuneUp tries to make it contiguous it does so by creating a temporary file and copying the current file to it. It then deletes the old file and renames it -- this way, if something happens during the process the old file isn't destroyed. Unfortunately, if the file is locked, the attempt to delete it fails and the other copy of the file is left around. Easy answer: unlock any file that TuneUp says it can't defragment in the log (you do read the logs, don't you?) and run TuneUp again. Then lock it again. One strange note on this: TuneUp can't defragment files that are locked, but it will happily relocate locked files to optimize disk space if they're contiguous.
