I believe that you can receive free from QMS a modified version
of the laserwriter drivers that will force the proper page
setup.  Personally, I have kept the standard Apple furnished
V6.0.x laser drivers and just remember to not use the top/bottom
"forbidden area" (there is also a forbidden right/left area).

I live with this because I (and my group of Mac users that I
support) seldom use the QMS.  Collectively
we use the hell out of it, but using the special laser driver
would become a maintenance headache, i.e. Apple comes out
with the latest driver and I have to call QMS to get it.  The
way I live, we just use the standard Apple driver and live
with a (very small) inconvience.  

I hope that someday
in the near future the apple driver will have a custom
area (like the backfax driver for the applefax modem) that
will allow you to define your own page size.  This
would be very useful for small stationary, for example.
This would also fix your problem with the QMS.
