Help!

I'm looking for examples and documentation on how to write exception
handlers.  I've seen the paper "The Mach Exception Handling Facility"
circa April 1988, but the paper is not very detailed.  For example, I
can't find any documentation on how to use exc_server() (libmach) and
the user supplied catch_exception_raise() procedures.
