Can you say where?  I've looked over the entire form and can't find anything
(except a thing on the front that says "Sales tax is charged and is based
on upon the applicable rate in the state to which the order is shipped.
Please contact your Education Market Specialist for assistance.").  If it
helps, the back of the form (lower-left hand corner) has a line that says
RJG/1094:smk (which is presumable the form # or something similar).
