The form I have doesn't provide a form number, but it came with the May 8th
Price List.  At the place where it lists Sales Tax, the fine print says:
Sales tax is charged in selected states and is based on the applicable rate in
the state to which the order is shipped.  Individual purchases in CA, IL, GA,
PA and VA must include applicable tax.  School orders in CA must also add
applicable tax.  Please contact your Commodore Education Market Specialist for
assistance.  All sales are final.  Prices, product availability, terms of the
offer, and conditions are subject to change without notice.  Changes to the
Education Purchace Price are not retroactive.
