Formatting the disk on the Rainbow is counterproductive. The formats are different. When using P/OS, the installation procedure automatically formats the hard disk for you. I don't know if the RT-11 install procedure does the same thing or not. DEC originally shipped all its hard disks for the Pro pre-formatted (um, actually, not "originally", but "at one time") so the version of RT-11 that you're working with may assume a pre-formatted disk.
