See chapter 19 (Azimuthal Equidistant Projection) in "Map Projections
Used by the U.S. Geological Survey" - USGS bulletin #1532 - available
from the main USGS offices - I got mine from Denver.

Alternately, if all you really want is the map, there are advertisements
in amateur radio publications of folks who'll make such a map centered
on any location on earth.
