It's the VIP (VGA Improved Performance) VGA card from ATI Technologies, Inc. It haven't tried it with a CGA monitor (I don't have one), but it has a 15-pin plug for an analog VGA monitor AND a 9-pin plug for a TTL monitor (CGA or monochrome).
