The driver for the disk supplies the icon in response to a _Control call with a csCode of 21 or 22. The icon returned for csCode 21 is meant to represent the volume media; the icon returned for csCode=22 is meant to represent the physical drive. This is documented in IM 5, pp. 469-471...
