The answer lies in TUGboat where there have been two articles on
the topic, one by Donald Knuth, the other by Adrian Clarke. These
articles appeared in TUGboat 8#2 and 8#3 respectively. In the UK,
you can get back issues through Aston University: contact Peter
Abbott (abbottp@uk.ac.aston) for details. I believe Aston may
also have the halftoning fonts and macros.
