When I got my guide dog from the Guide Dog Foundation in Smithtown, Long 
Island, New York, there was a branch of Canine Companions for 
Independence near us.  I don't have their address but if nobody else has 
it here, you could call Smithtown information at 1-516-555-1212 and get 
the phone number and I'm sure the school could give you the address for 
their headquarters in California.
