The one I used is made by:

        Rainier Technologies Corp.
        (sorry, I don't have a street)
        Redmond, WA  98052
        
        Model TB-201, Voice/Data Switch

I know of several of these in use and the only problem is if they are
on a bad power line.  The solution is to plug them in to the good EMI
filter you have for your PBX.
