I would assume that this would minimally include a mention of the authors and the entry's accession number. Also, on one of your other points, as humble as it may be, simple checks of things such as ORF's and the presence of vector sequences do sometimes reveal problems with the data. Obviously it is hard to detect a single base change due to the misreading of a gel, but this is where other experimental confirmation (e.g., is the protein sequence consistent?) would eventually be needed.
