Can anyone tell me how to configure a DHQ11 into an Ultrix 3.1
system?  This is running on a VAXStation II.  

I need to know what "device" entry goes into the system configuration
file and what /etc/MAKEDEV will want to know.  I know what the csr and
vectors are.
