His problem is surely that the clip is not on properly. It needs to be *VERY* *VERY* tight - a sharp tug should not pull it off. The pins need to be VERY VERY DEAD LEVEL with each other. Use a ruler. Squint along the pins. A single pin out of alignment will cause the thing to fail. I was in your friend's situation for some time. I would not believe that this was the problem. But it is.
