a few questions. What did he use to low-level the drive, how did the install
fail?

Some controllers forget about the existance of any other os other than
MS-DOS and mark the ENTIRE track bad when they find a bad sector. This
will add 26 entries (26 sectors/track) into /etc/partitions on the uport
install quickly blowing the 64 entry limit.
