There is a program called "outspoken" which is a screen reader for the 
mac.  It is available from Berkley Systems in California.  I don't have 
the address but if you want, I can try to get it for you.   I've seen the 
program.  It's quite good.  It allows full screen access for most 
applications and it would be able to read text files.
It's not cheap--it costs $395.  But so far it's the only program I know 
of for the mac that would allow access to the screen.
