I got their little flyer, and I said, "Neat, I think I'll get this. I've been waiting for this for a while." So I proceed to fill the thing out. Then I set it down. Later, I decide, "Hey, it's time to write the check for this, and send it out." Now I can't find the flyer. Where and how much money do I need to send?
