Sounds like a DuoDisk controller to me--I have one in my IIe.  (The DuoDisk,
for those who are fairly new to Apple IIs, was two 5.25" mechanisms in one
box that is quite at home sitting atop a IIe.)  BTW, if you have Disk II or
compatible drives laying around, it is possible to adapt the DuoDisk 
controller to drive Disk IIs.  (The converse--using a Disk II controller
with a DuoDisk--is also possible.)  _Open-Apple_ did an article about making
the adapter cables; if there's interest, I'll dig it up and summarize here.
