I posted this a few weeks ago but got no response, so I'll try again.
 
For a program I'm writing I need to know how to toggle blinking and
underline modes, and how to detect vertical retrace.
I now have this information for all popular video cards with the
exception of the CGA.  If some kind soul could please tell me what
port addresses and bits to use I would be very happy.  It would be
all I need to make the program work with CGA and would save me
getting a book.
