SmartForm from Claris does exactly what you want. Apple uses it extensively: blank forms are kept on file servers and on AppleLink, so you just download the form you need, fill it in, and either e-mail it to where it goes, or print it down and hand in the hardcopy. I know nothing about pricing. Apple has a site license for it.
