I've written a cdev that takes care of 3) (except Undo), 4) (no remapping),
and 5). It also lets you select (most) buttons from the keyboard. I've sent
it to sumex and comp.binaries.mac, and it is currently available from
pollux.lu.se (130.235.132.89) in pub/mac/cdev as DialogFilter.hqx.
