Some how patch create "op.s.rej".
It turned out the header of "op.s" contains this line :

# $Header: op.subst,v 3.0.1.1 90/02/28 18:37:30 lwall Locked $
              ^^^^^
While patch was expecting :

# $Header: op.s,v 3.0.1.1 90/02/28 18:37:30 lwall Locked $
              ^
Change that and "make test" will pass.
