At the moment I'm using a program called Startup. It's quite good, but I would like something a bit more flexible. A C interface would be very nice as well. I'm running an STe with 2meg of memory, so I'm copying lots of things to a ram disk when I boot, and I want to have a large degree of flexibility about what gets copied each time.
