The R4 twm has always appeared to handle colormap setting the ICCCM way. twm expects a client program to set a colormap attribute on a window with XSetWindowColormap or equiv. The twm distributed in the core R4 distribution (patch level 0) allowed clients to set colormaps only by the ICCCM way. If a client tried to do an XInstallColormap, twm would sense the colormap change event and then install the system colormap regardless of the colormap attribute associated with the window. To maintain backward compatibility with window managers, ximage set colormaps the ICCCM way and the pre ICCCM way. This didn't work with the old twm. Patch level 8 for the R4 distribution fixes the above mentioned problem so that clients may do this wm backward compatibility thing with colormaps setting. I have not experienced any problems with twm patch level 8.
