VAXstation 3200, qbus 9-track tape, Ultrix 4.0

When I upgraded to Ultrix 4.0 (from 3.1) our tape drive started acting
possessed.  It hangs the system, just shuts off sometimes, and sometimes
reading garbage off of the tape - sometimes right off the end of the tape.
Do any of these problems sound familiar?  I really need to get this fixed -
We're down to one 9-track tape drive which is pretty much constantly in use.
Any help would be greatly appriciated.
