It will appear on comp.sources.x soon and is on expo.lcs.mit.edu [18.30.0.212] in the contrib/ directory as bdftops-1.0.tar.Z. It works with both variable width and fixed width fonts, although some hand tuning of the Postscript might be required(not much). The man page gives a short, limited example on hand tuning.
