You could try getting the adobe screen fonts from info-mac/fonts/ on
sumex-aim.stanford.edu.  These fonts might have more accurate font
metric information than the fonts you are working with, especially for
italics, bolds, and bold italics.

Also, be sure you don't have any "font substitution" going on when you
go to print (make sure this box is not checked).  If fonts are
substituted, you are certain to get such bizarre results, since the
widths are different.
