I can't say what you should use, but I had success using Metaware High C and the Phar Lap assembler and linker. Be prepared to do some heavy-duty system-level development to establish code and data communication across the USE32 and USE16 segments. I enjoyed it myself -- a good opportunity to work with native 386 code.
