Since I have MacsBug, I can see that it bombs at exactly the same location all the time if the above conditions are met. I've got a Mac IIx, 8MB RAM and running System 6.0.5. I've tried reinstalling everything and tried pristine, INIT/DA-less configurations to no avail. I've talked to SuperMac and Fifth Generation Systems and have even sent two detailed reports to Fifth Generation Systems.
