Personally, I'd use Dpaint and then ZapIcon or something to convert the
brush to an icon.  The time you lose in the conversion is paid back in
the time you save in not having to deal with an icon editor.  
 
The world (or my :tools directory) still needs a better converter,
but I've never been impressed with any of the icon editors yet.
