Of course TeX cannot do this: you are asking for a new glyph: a new character shape. This is a job for a completely different program. The task of creating a new font that is exactly like an old one, except that the left quote mark is upside down, is trivial. Just copy an existing Metafont program, change its name, and change the left and right quotes from being rotations of each other to being reflections of each other. All that aside, I feel compelled to mention that I find most upside down left quotes grotesque.
